Special Population Tools: Spanish Zones Tools for Asthma, Heart Failure, COPD, and Diabetes

These tools are the Spanish translations of the disease management zones which provide patients with easy to follow guidelines for assessing signs and symptoms of their disease process and exacerbations. The tools also advise patients on when to report problems to home health agencies and physicians. The tools include disease management for asthma, diabetes, COPD, and heart failure.
