Tools: Resource Package - Improving Acute Care Hospitalization

This resource is intended to support the outcome-based quality improvement (OBQI) process by providing evidence-based actions, tools, and resources related to this topic. It is not intended as a prescriptive, ready-to-use program of care, but rather as a resource for guiding an agency's process of care investigation and developing a plan of action.

The actions included in this resource were identified from a review and synthesis of recommendations from evidence-based sources. Evidence sources are seldom specific to home health care; therefore, the recommendations, tools, and resources most appropriate and adaptable to the home health setting were identified.

Steps to enable JavaScript in Internet Explorer
  1. Click on the Tools menu option at the top of the page.
  2. Select Internet Options from the drop-down menu.
  3. From the dialog box, choose the Security tab.
  4. Click on Custom Level.
  5. Set Active Scripting to Enable.
  6. Click OK to close the custom settings dialog.
  7. Click OK to close the Options dialog.
