Tools: Resource Package - Oral Medications: Quality Medication Administration Project
This resource combines best practice tools with supporting resources to offer home health agencies a complete guide for improving patient oral medication management. Best practice tools assist agencies in assessing medication use, handling medication simplification, finding medication simplification tools, and managing oral medications. The materials are differentiated between those for Quality Improvement Organizations (QIOs) and those for home health agencies (HHAs). The QIO materials consist of: Recruitment, Technical Expert Panel, Pre work, Best Practice Tools, and Supporting Resources. The HHA materials consist of Best Practice Tools, and Supporting Resources.

Background
The Quality Medication Administration Project was the result of a change package implemented by twenty-two Pennsylvania home care agencies with the use of the Outcome Based Quality Improvement Process and the Institute for Healthcare Improvement (IHI) Breakthrough Series (BTS) Collaborative.
Directions
Home health agencies would use this resource to improve patient management of oral medications, ensuring that patients are both taking their medications as prescribed and are aware of medications that are potentially ineffective and/or harmful. The resource is also used to educate all levels of home health agency staff on medication assessment and instruction protocols.
