Tools: Resource Package - Improving Dyspnea

This resource is intended to support the outcome-based quality improvement (OBQI) process by providing evidence-based actions, tools and resources related to improving dyspnea. It is not intended as a prescriptive, ready-to-use program of care, but rather as a resource for guiding an agency's process of care investigation and developing a plan of action. The focus of this resource is on identifying strategies and actions and associated tools and resources related to promoting patient self-management and implementing evidence-based practices and guidelines. Fewer evidence-based actions were identified in the other three areas.

It is not expected nor intended that home health agencies try to adopt all of the strategies and actions offered, but rather only those that they deem appropriate based on the results of their process-of-care investigation. While all the identified strategies and actions can facilitate improvement, any one action may not directly lead to a change in an agency's outcome rate.

The actions included in this resource were identified from a review and synthesis of recommendations from evidence-based sources. Evidence sources are seldom specific to home health care; therefore, the recommendations, tools, and resources most appropriate and adaptable to the home health setting were identified.
