Tools: Facility Assessment Checklist for Pain

These checklists are to be used by nursing home staff to identify areas for improvement within the existing processes of care in a facility. Checklists are the first step in a quality improvement project. The checklists cover the following areas: screening, assessment/reassessment, staff training, care plan development, monitoring/tracking, policies and procedures, and staff education and training.

Background

While providing onsite quality improvement consultation in nursing homes, it was determined that providers assumed their current care processes were consistent and adhered to current guidelines. Through chart audit, it was determined that this was not necessarily true, so the facility assessment checklists were developed to assist in assessing current practice and identifying areas for improvement.
